The mission of the Cobb County School Health Services is to strengthen and facilitate the educational process through improvement and protection of the health status of all students. School nurses work as a team with school administrators, teachers, counselors, school social workers, families and community to identify and assist each student to reach their maximum state of well-being.
Areas of Responsibility
Medication Forms: If your student requires prescription medication at school, the following form(s) will be required. All prescription medication must be administered through the clinic. Please discuss any medication concerns or questions with the nurse.
Medical Forms: if your student has any chronic medical condition, the following forms are requested for clinic use and appropriate treatment/intervention and care.

Asthma; Diabetes; Seizures;
Allergies; Food Allergies;
Chronic Illness; Sickle Cell; etc.
Clinic Card: Please complete a clinic card annually for each student. These are routinely sent home at the start of each year in your "Welcome Packet' as well as upon registration throughout the school year.
